An integral part of a comprehensive Health and Safety Management system, each permit form records hazards and precautions relevant to the specific job. These forms contain three parts, the original form plus two carbon copies as well as a card backing insert for separating the carbon copies. Supplied in packs of 10.

2. Hazards and Precautions to be taken (Primary Hazards, Other precautions, Other safety equipment required and Security checks timesheet).
